Join us for the Health Fair and Sports Physicals event on May 29, 2025, at DGF Gymnasium, with paperwork due by May 16, 2025, for middle and high school students.
We are excited to announce that the annual Health Fair and Sports Physicals event will take place on Thursday, May 29, 2025, from 0900 to 1400 at the DGF Gymnasium. This event is crucial for all middle and high school students who plan to participate in sports activities during the upcoming 2025-2026 school year.
Important Dates and Information:
- Paperwork Availability: Starting April 7, 2025, paperwork can be picked up from the school nurse or the MHP Clinic.
- Submission Deadline: All paperwork must be completed and turned in to the school nurse no later than May 16, 2025.
- Screening Appointments: Students who submit their paperwork on time will receive a screening appointment call from the medical team prior to the event. Note that the phone visit will be with the parent or guardian of the student, and the student does not need to be present for this call.
